By signing and dating an Informed Consent & Waiver form all customers voluntarily and knowingly consent to participate in massage therapy services (collectively “Services”) provided to them by Suriya Spa Ltd (Trading as Peppermint Massage & Beauty). Customers must certify that they have read, understand, and agree to abide by the full policies of Peppermint Massage & Beauty, and thereby assume all risks involved in their voluntary participation in the Services. By signing they have understood the treatment that they are to receive and confirm that they are willing to proceed without confirmation from their own GP or Consultant.

 

By signing and dating an Informed Consent & Waiver form all customers thereby WAIVE, RELEASE, AND FOREVER DISCHARGE Suriya Spa Ltd, their affiliates, managers, members, agents, staff, heirs, representatives, and assigns, from any and all claims of negligence, injuries, or losses that the customer may incur as a result of their participation in the Services. They agree to indemnify and hold harmless Suriya Spa Ltd against any and all claims, suits, or actions of any kind for liability, damages, or compensation brought by them or anyone on their behalf, including attorney’s fees and any related costs. They affirm that they are entering into this waiver and release of liability fully of their own volition.
